What your audits have found, how serious and how old it is, and whether it keeps coming back — open findings, aging, repeats and accepted risk.

Audit Findings Report
The Audit findings report shows what your audits have found, how serious it is, how old it is and whether it keeps coming back. It is a state report: figures are as of today, and the as-of banner says so.
Headline Figures
- Findings open — reported in a draft or final report and not yet closed.
- Findings at the top of their audit type's rating scale.
- Findings open more than a set number of days since the final report was issued (180 by default; an administrator can change the threshold, see Audit Settings).
- Repeat findings still open.
- Observations.
- Findings still in draft during fieldwork.
- Findings closed because a senior official accepted the risk, with who and when.
In the Period
The findings raised and closed in the period (verified, risk accepted, superseded by a repeat, withdrawn) and the repeat rate of the audits issued in the period.
Breakdowns
- Open findings by rating. One table per rating scale when your audit types use different scales.
- Aging matrix. Days since the report was issued, by rating. Findings whose final report is not out yet have their own row.
- Other cuts. By criterion, by root cause, by audited subject and by audit type.
- Repeat findings. A list with the earlier finding each repeats and how many times the problem has recurred.
Good practice the audits recognized is counted on its own and never mixed into the findings. A finding the auditor withdrew counts only as withdrawn.
Filters and Drill-down
The five audit reports share a filter bar: audit type, lead auditor, plan year and audited subject. Every headline figure is a button that lists the findings behind it (Show all clears it), and every row opens its audit. The report is hidden while the Audits board is off.
